Michael Yasick ADHD Scholarship

Each Scholarship Recipient will receive a one-time, $2,000 tuition payment with one (1) year of ADHD coaching services by the Edge Foundation (the ADHD coaching service has an approximate value of $4,400) (collectively, the "Scholarship"). A total of fifty-five (55) Scholarships will be awarded to recipients meeting the criteria designated in the Rules and Regulations below. Of the fifty-five (55) Scholarships, five (5) Scholarships shall be awarded only to recipients who are employees of the Scholarship Sponsor, a qualified Immediate Family member (as defined below), or qualified individual with whom the employee lives, who in each instance must provide a valid Shire Employee Number (each an "Employee Scholarship"). The five (5) Employee Scholarships shall be awarded to employees (or qualified Immediate Family members or those with whom they live) who are enrolled in, or accepted to, an eligible Undergraduate Program or Graduate Program at a Qualifying Institution. Employees of the Scholarship Sponsor (or their qualified Immediate Family members) are not eligible for the remaining fifty (50) Scholarships (each a "National Scholarship"). Of the remaining fifty (50) Scholarships, all of which are National Scholarships, ten (10) National Scholarships shall be awarded to recipients who are accepted to, or enrolled in, an eligible Graduate Program at a Qualifying Institution (as defined below). The remaining forty (40) National Scholarships shall be awarded to recipients who are accepted to, or enrolled in, an eligible Undergraduate Program at a Qualifying Institution.

Limit one (1) Scholarship award per eligible award recipient.

The likelihood of an applicant being selected as a Scholarship Recipient will depend on the number and caliber of Applications received over the duration of the Program.

Eligibility Requirements

To be eligible for a National Scholarship or an Employee Scholarship, you must meet the following criteria:

A) Be accepted to, or enrolled in a 2- or 4-year Undergraduate Program or a Graduate Program (of one or more years in length) at an accredited college, university, trade school, technical school, or vocational school located in the United States as defined by the Internal Revenue Code of 1986 (as amended from time to time) (a "Qualifying Institution"). Please visit http://www.ope.ed.gov/accreditation/Search.aspx to confirm Qualifying Institutions. For purposes of eligibility requirements of this Scholarship, an Undergraduate Program consists of a 2-year associate's degree program or a 4-year bachelor's degree program; whereas, a Graduate Program consists of an advanced, graduate academic degree program of one (1) or more years duration (undertaken or to be undertaken by an applicant who has already obtained a bachelor's degree) including but not limited to, a master's degree program or a PhD or doctorate degree program. To be eligible for a National Scholarship or an Employee Scholarship, the applicant must be enrolled or accepted to an Undergraduate Program or a Graduate Program at a Qualifying Institution.

B) Be a legal resident of the fifty (50) United States or the District of Columbia.

C) At the time of submitting the Application, each applicant must be diagnosed with ADHD and under the care of a licensed health care provider for ADHD, although no specific future or ongoing plan of management or treatment for applicant's ADHD is required in order for applicant to be eligible to receive a Scholarship.

D) Each potential award recipient must provide proof of age (a notarized, legible copy of a birth certificate, driver's license, passport, military identification card, or other identification card issued by the state or federal governments containing your date of birth) with the Acceptance Package (as defined below).

E) All applicants selected as potential award recipients must have a licensed health care professional confirm their ADHD diagnosis by completing a Confirmation of Diagnosis and submitting it to Shire. Shire will mail potential award recipients the Confirmation of Diagnosis form. Potential award recipients who fail to provide a Confirmation of Diagnosis by a licensed health care professional will not receive an award.
