ChiroHealthUSA Foxworth Family Scholarship
The Foxworth Family Chiropractic Scholarship is an achievement-based scholarship awarded to students currently enrolled in Chiropractic College. Students are recognized for their capacity to lead and serve, and their commitment to making an impact in their communities and the profession.
The Foxworth Family Chiropractic Scholarship is fully funded by ChiroHealthUSA in honor of President Dr. Ray Foxworth's parents, Drs. Betty Pace Mathews and Charles Vernon Mathews. The award includes a $10,000 scholarship for school expenses, $5,000 in cash to help offset living expenses, and an additional $10,000 donation to the chiropractic college where the recipient of the scholarship is enrolled.
A) Unless graduating in the mean time, the recipient must remain a full-time student, enrolled in a Doctorate of Chiropractic program at an accredited Chiropractic college or university in the U.S. during the entire academic year of the award, or they shall forfeit the award.
B) Minimum 2.7 GPA.
C) Two recommendations submitted through the online application. Contacts listed for recommendation will be sent a link to complete an online questionnaire.
D) Recipient must agree to keep the award confidential until it is announced at The FCA National Convention in Orlando, Florida, on August 23, 2019, or they shall forfeit the award.
E) Applicants may not be family members of employees, officers, or owners of ChiroHealthUSA.
