CPAC Foundation presents annual awards to Ontario high school students who have completed grade 10, 11 or 12, and who have demonstrated outstanding academic achievement and active community involvement. Applicants are evaluated based on academic records, volunteer/community work, leadership and impact. There will be a maximum of four $1,000 awards for students who have completed grade 10, 11 or 12 studies, with a maximum of two awards for each grade. To be considered, students must submit a completed online Award Application by January 15, 2019.

Eligibility Requirements:
You are eligible to apply for a CPAC Award if:
A) You are a Canadian citizen or permanent resident of Canada;
B) You are a student who have completed grade 10, 11, or 12 from a recognized Ontario high school;
C) You have demonstrated academic excellence by achieving a minimum average of 80% in your most recent academic year;
D) You have engaged in volunteer activities and have demonstrated outstanding commitment to the community;
E) You have been enrolled full-time for the previous school year and you are currently enrolled as a full-time student for the 2018-19 school year (minimum 3/4 courses per semester);
F) You are NOT a previous CPAC Award recipients.
Applicants are required to submit all supporting documentation along with their applications. Failure to include all documents required in your application may result in the refusal of your application.
