For years, Belltown Spine & Wellness has been committed to providing care and comfort to the community. Now, we want to help in a slightly different way: A $1000 scholarship for one promising student.
Additional criteria:
This contest is open only to citizens and legal residents of the United States over the age of 13. Entrants must be enrolled as a high school senior, or a student enrolled in any undergraduate or graduate college program. We will affirmatively verify the eligibility of the winning entrant, and he or she may have to sign an affidavit confirming eligibility.
The Contest
All applicants should fill out the scholarship application and include their full name and mailing address with their submission. The submission should include a personal statement, well as an essay on the following topic:

The Scholarship Prize
The winning entrant will receive a check in the amount of $1,000.00 (one thousand) U.S. dollars (which may be used without restriction in the winning entrant’s sole discretion, but that we hope will contribute to furthering his or her education). The Scholarship Prize will be sent to the winner by U.S. First Class Mail or by other means of transmission selected by Belltown Spine & Wellness in its discretion, within five (5) business days of Belltown Spine & Wellness selection of the winning entry.
